Welcome to our office

We look forward to meeting you.  Our office is dedicated to helping patients with  jaw disorders, or with pain in the face, jaw, head, or neck.  We are committed to supporting you in resolving your symptoms and concerns with the minimal treatment necessary. 

To make an appointment, please call 801-261-9155.  We are open M-Th from 7:00 am to 3:00 pm. 

Prior to your appointment, we strongly suggest that you review all in the information on this site under the heading “Patient Education”.  This will help you understand the nature of jaw problems.  It will also prepare you to better understand the details of your condition at the time of your consultation in our office.

Your initial appointment will take approximately 90 to120 minutes.  We require that you complete a comprehensive health questionnaire and symptom survey prior to your appointment.  If you fail to do so, you will need to arrive at least 30 minutes prior to your scheduled appointment time to complete this lengthy 8-page document.  You can download the form here:  Patient Information Form     A Cat Scan of your jaws and jaw joints will be obtained, to help diagnose your problems. Your vital signs will also be taken.  Dr. Guinn will then interview you, in order to understand your concerns and symptoms.  He will then perform an examination, followed by a detailed consultation with you.  To understand everything that takes place at your examination appointments, please click here.

If you are currently using a bite guard or splint, please bring it with you to the examination appointment.

Because of the length of these appointments, we ask that you do not bring young children with you.

Failed Appointment Policy

Anyone arriving more than 15 minutes late will be considered to have failed their appointment.   Please allow extra travel time if you will be arriving before 9:00 am, as we are located on a busy street (Redwood Rd).  You will find that our office does an excellent job running "on time".  This can only be accomplished if YOU are on time for all of your appointments.

Because so much time is reserved for you exclusively for this lengthy first appointment, it is critical that you do not fail this appointment.   We require 48 hours notice in the event you are not able to keep your appointment.  Patients failing to provide this notice are subject to the following conditions:  Another appointment will not be made for you until you have sent us a check for the full amount of the first visit, in addition to a missed appointment fee.  Once your check has cleared the bank, we will call you to set up another appointment.

Insurance

Most insurance companies do not cover treatment for jaw disorders.  Many have a specific exclusion clause that denies benefits for jaw problems.  We cannot provide services contingent on insurance reimbursement.  You will be directly responsible for all charges associated with your visits.  Payment is due at the time of service.  All major forms of payment are accepted.  We will provide you with the forms to submit for dental insurance reimbursement (Dr. Guinn is a dentist, so we are unable to provide you with medical claim forms).

Lodging For Out-Of-State Patients

If you do not have a friend or relative to stay with in Salt Lake while visiting our office, we can recommend a facility which offers a discount when the purpose of the visit in for medical reasons.  This is the Crystal Inn Hotel and Suites, located at 2254 City Center St, West Valley City, UT  84119  Phone: 801-736-2000  This facility is less than 10 minutes from our office.  It is located near the junction of I-215 and 3500 South.

Privacy Assurance Statement

This office respects your privacy.  The privacy of your health information is our utmost concern.  This office has always complied with any applicable state and federal privacy laws and will continue to do so.  Everyone in this office maintains high standards of discretion and professionalism.  Your health information is not used for any marketing purposes.  All staff members receive thorough privacy training and are expected to maintain high levels of professionalism.  In communicating with other doctors about your care, only required information is discussed.
